• Attested photo copy of birth certificate (Municipal/ Block/ Panchayat).
• Transfer certificate from previous school along with report card were applicable.
• Six copies of passport size photograph of the child.
• Four copies of latest photograph of Father and Mother/ Guardian.
• Duly filled Application Form for admission.
